Community Resources

In order to assist with the needs of our patient population, we have identified community resources that can be utilized by our healthcare professionals to coordinate care as well as connect our patients with vital information to help with important health decisions and assist with self-care.

  • Advance Directives Resources
    This link will take you to the Tennessee state website for Advance Directive Resources. Advance Directives are health care decisions made in advance of trauma or major illness such as Advance Care Plan, Living Will, and Medical Power of Attorney. This link contains all the resources, including forms and other information, you might need in order let your family and doctor know your wishes in the event that you are unable to make decisions for yourself.
  • Alzheimer's Tennessee
    Alzheimer's Tennessee is a non-profit organization which provides suport for those affected by Alzheimer's and related diseases. Resources provided include family support, community education, and advocation of needs and rights of Alzheimer's patients as well as resarch for its prevention, cure, and treatment.
  • Knoxville, TN Meals-on-Wheels and & Senior Meals Programs
    This link provides resources to find meal programs for seniors in East Tennessee. If you cannot find a program near you, please use the link at the bottom of their page to contact Meal Call and they will attempt to help you the best they can. Meal Call is an organization of volunteers with the purpose of helping people find local senior meal programs, recruit volunteers, and help businesses develop corporate sponsorship and volunteer programs for senior meal programs.
