What is Follow My Health?
The FollowMyHealth patient portal is a secure, online patient portal service that allows you, as a patient, to communicate easily with your Summit Medical Group Provider.
Is there a fee to use FollowMyHealth?
No. FollowMyHealth is a service provided free of charge to all Summit Medical Group patients.
Is my information safe?
Yes. Your private health information is protected and maintained securely by Summit Medical Group. There are a number of security measures and standards in place (HIPAA, PCI, and other regulatory and compliance agencies) to ensure that your data is protected as you use Follow My Health.
The FollowMyHealth patient authorization indicates once my medical records are disclosed by Summit Medical Group to FollowMyHealth, they are no longer protected by the HIPAA Privacy Rule and the records may be disclosed further?
Summit Medical Group and FollowMyHealth are covered under HIPAA to allow for the sharing of data between Summit Medical Group and FolllowMyHealth; however if the information is disclosed by the patient, i.e, printed or accessed in public area where others can view the personal health record, HIPAA does not apply.
How do I sign up?
On the FollowMyHealth login page, click "Create Account" and follow the onscreen instructions. Make sure you choose Summit Medical Group, Weisgarber, as your connection, enter 37909 zip code.
After registration, you will receive two emails, one welcoming you to FollowMyHealth, and one confirming connections with Summit.
Does FollowMyHealth share my email address with anyone?
No. FollowMyHealth does not distribute email addresses to third parties. However, please note that if you are using an email service which scans email for content, you might experience a rise in certain types of bulk mail, depending on your provider.
What if I forgot my user ID or password?
Use the "Forgot Your User ID/Forgot Your Password" links on the main FollowMyHealth log-in page, which are located under the user ID and password fields. You will need to confirm certain information to have your user ID sent to you or to reset your password.
